People always ask us “How long should this last?” We are happy to provide you this written warranty for the investment you have made in preserving your oral health, something few offices offer. Our goal is not only to treat your current dental problems but also to educate you in preventing further tooth decay. Instead of going every few years for “drill, fill, and bill” when things hurt or break, let’s try preventative dentistry. You can prevent most dental disease if you spend adequate time on oral hygiene at home and let your dentist or hygienist professionally clean your teeth, check for decay, and take necessary x-rays. This warranty agreement is offered to patients that follow a regular hygiene schedule in accordance with our recommended treatment plan. Our warranties must be null and void if we don’t see you for your regular recare visits. With recare visits, your teeth and gums are winners! Let us remind you that the final ownership of your oral health rests on you.
Sealants
We will repair or replace sealants for a period of two years from the placement date. If decay occurs on the chewing surface within two years of placement, the replacement filling will be done at no charge.
Amalgams(silver fillings) and Composites(white fillings)
We will repair or replace at no charge any fillings that fail or break for a period of two years after placement. If a tooth with a filling done by Dr. Hwang breaks within two years of placement and requires more extensive treatment, the fee for the filling will be credited towards whatever work needs to be done.
Lab fabricated crowns, bridges, or veneers
We warranty these restorations for a period of 2 years. We will replace them at no charge if they break or are lost. This does not include injury, accidents, or chewing ice or non food items. If the tooth is broken and non-restorable, we will credit the fee for the crown, bridge, or veneer towards whatever work needs to be done. We cannot predict if and/or when gum and nerve treatments may be needed on the covered tooth. If needed, they would require additional fees.
Dentures and partial dentures
We will repair dentures and partials for a period of 2 years from the placement date if a tooth chips, breaks, or a flange breaks under normal use. We will do adjustments as needed for no charge for a period of 6 months from the placement date. We will do one reline if needed at no charge within a period of 1 year from the placement date. This warranty will not apply to any accidents and dropping your dentures. Full denture patients must be seen once every 12 months in our office. Patients with any remaining natural teeth must be seen at prescribed recall appointments in our office.
Conditions
- In order to keep this warranty valid, you must:
- Maintain your recare visits (3, 4, or 6 months) in our office for professional exam, x-rays, and cleaning. You must be seen within 30 days after your due date according to your recommended schedule. Failure to maintain your recare schedule will render this warranty null and void.
- keep account in good standing(i.e. account not sent to collections)
- complete necessary dental treatments as prescribed by our office to prevent gum infection, nerve infection, and tooth decay, which have a tendency to spread from tooth to tooth.
- This does not cover damage to teeth or prostheses caused by trauma(ex. Auto accidents, sports injuries, etc).
- This does not cover damage caused by chewing ice or non-food items(ex. Bottle caps).
- This does not cover any work done by Dr. Carter.
- This does not cover procedures done that are not the ideal recommended treatment advised by Dr. Hwang. For example, if you are advised that a tooth needs a crown and you elect to try a filling, that filling will not be covered by this warranty.
"Amnesty" offer:Any patient who is overdue for a recare visit and comes in for a recare visit before February 1, 2009 will have this warranty apply to whatever work was done by Dr. Hwang. After this date, we will not be able to warranty any work done for any patient who is overdue for their recare visit.
